Path to Zero Content and Calendar

The Path to Zero cohort supports community teams through monthly office hours and peer learning sessions focused on emerging best practices in harnessing data, using quality improvement as a framework for change, and case conferencing. Over the course of this year, you will be supported by your peer communities and a dedicated coaching team who will support you in identifying which critical elements need strengthening in YOUR community and bring tools and frameworks to support your progress.

Data Office Hours: Data Reporting Orientation

Improvement Science and Systems Theory offer critical lenses through which we can better understand how a community’s homeless prevention & response system is performing, why, and what changes might be tested to improve it. 

In this session, we introduce key concepts and frameworks to supply the knowledge and motivation you need to adopt quality improvement methods into your way of working. This intellectual foundation will set the stage for diving deeper into the Model for Improvement and its application in your daily work during sessions two and three of this series.

Quality Improvement 2: The Model for Improvement

During this second session, we introduced the Model for Improvement (MFI), which is a simple, well-tested model for preparing, scoping, and managing efforts to improve system performance. First developed for application in manufacturing and other “closed” systems, use of the MFI has since spread over the past few decades to more complex, “open” systems, such as health care, education, and social services, and to great effect. Community  Solutions has utilized the MFI to drive our improvement initiatives since the 100,000 Homes Campaign.

Quality Improvement 3: Plan, Do, Study, Act

During our third session, we shifted to focusing on how it may look in practice for a community-level initiative, agency, program, or frontline staff to apply the Model for Improvement in your daily work of resolving and reducing homelessness. We introduced tools and resources you can use to start applying the MFI anytime, as well as educate your colleagues and partners in order to build will in your community for the shift in mindsets, behaviors, and operations that comes with embracing a culture of improvement.

Case Conferencing Academy

Please review this self-paced, asynchronous curriculum prior to August 2. We recommend setting aside at least 45 minutes for each module to watch the content and engage with the tools. Each module provides a set of practices and resources that you can use to begin implementing improvements to your current system, so we recommend spacing out this content over a month or more so that you have time to make improvements to your case conferencing practices.
